REGISTERED NURSE CASE MANAGER POSITION

PMN Consulting, Inc., established in 2000, is an independent nurse case management company providing a compassionate and experienced approach to managing medical care, from the routine to the complex and/or catastrophic. We coordinate a variety of medical and academic related services for adults and children with injuries resulting from motor vehicle accidents. Our job is to maximize our clients' independence and empower them (or their caregivers) to effectively manage their lives. This position requires a high degree of professionalism, confidentiality, compassion, and teamwork.

Major Responsibilities
-
Advocate, educate, and coordinate client care with interdisciplinary team to maximize outcomes and ensure continuity of care (i.e., medical providers, home care providers, behavioral health services, transportation, DME, educational services, and other needs as required).

Evaluate client and facilitate physician-directed Treatment Plan.

Identify client-centered goals.

Schedule and attend doctor appointments.

Document client/case activities through billing logs and submit monthly case management progress reports.

Excellent communication skills, both oral and written.

Knowledge of Auto No-Fault a plus.

Bilingual a plus.

Ability to establish and foster lasting relationships.

Experience in case management preferred, including: home care, discharge planning, TBI, Spinal Cord Injury, ICU/ER, pediatrics, or rehabilitation.
Requirements
-
Current and valid Michigan RN license with 5 plus years of experience.

Valid driver’s license and insurance.

Reliable transportation for local travel within the Metro Detroit Area.

Proficiency in technology (Microsoft Office Suite, smart phone, web research, etc.).
